Eligibility: Live or work in Polk County for at least three months, have no medical insurance, and have a family income that does not exceed 150 percent of the federal poverty guidelines. Fees: Free Intake Procedure: Call for screening appointment Languages Spoken: English, Spanish & Sign Language by request Service Hours: Monday: 8am-12 noon Tuesday - Friday: 8am-5pm Description: Staffed almost entirely by volunteers, the clinic offers routine primary and dental care. Pediatrics, cardiology, gynecology, dental care, eye and ear care, vaccinations, mental health counseling, pharmacy and general medicine are examples of the types of treatment offered by the clinic. LVIM does not provide emergency services.
