All eligible patients who apply for this discount program must provide proof of income. Patients are expected to pay appropriate co-pays at time of service. Hours: Monday, Tuesday, Thursday and Friday, 7:00 am - 5:00 pm Wednesday, 10:00 am - 5:00 pm
