PrimaryOne Health  provides preventive and operative dentistry for adults and children. Services included: preventive exams x-rays cleanings fluoride treatments sealants fillings and extractions (no wisdom teeth) Necessary root canals are performed only on the front teeth. Does not offer orthodontics such as braces. Eligibility: anyone Intake process: Telephone. Appointment required. Documents: photo id. If applicable, insurance cards. All currently prescribed medication. Proof of household income (2 current pay stubs, workman's comp income, SSI, unemployment benefit statement, letter from employer if paid in cash, or current Medicaid enrollment ID). Fees: Sliding fee scale for uninsured, Franklin County residents. Minimum payment of $40 per visit. Private insurance. Medicare/medicaid.
